How should the 3 Ports shown in the figure be dealt with?
All that I could find is the Middle one Should be connected to Oil Catch Can.
What is advisable for the other two? Should they be blocked or left open or interconnected?

Middle is the crank case vent, put it to your catch can and make sure your catch can has a vent. The outer two are secondary air injection. We block them off, normally they take filtered air from your airbox on the bike and pass it through to the exhaust to help complete the combustion for emissions. I doubt they have any effect on power whether you block them off or leave them open.
